Working Title: Access Clinician
Program: Access
Hiring Salary Range: $38,086 - $50,160
Position is 2nd Shift (3:00pm - 11:00pm, Monday-Friday)
Proposed Start Date: Immediately

Primary Purpose of Position:
The primary purpose of this position is to provide telephone screening and triage services such as assessment of consumer's problems and needs; determine the urgency of the situation and safety concerns; provide brief crisis intervention; determine likely target population eligibility; advise the consumer of the choice of available providers, and schedule a specific appointment for assessment with the provider the consumer has chosen.

Detailed Description of Duties:
Telephone screening and triage to include: assessment of consumer's problems and needs (this includes both children and adults from all three disability areas). This position will determine the urgency of the situation and assess safety issues. Where safety is at risk, the employee will initiate immediate intervention via law enforcement, emergency medical assistance, involuntary commitment, etc.

Determines eligibility for assessment and inform the consumer of the choice of available providers. Schedules a specific appointment with the provider the consumer has chosen.

Provides brief crisis intervention counseling and/or support when indicated to facilitate the assessment process, encourage compliance, address safety issues, etc. Clinical functions will be performed by the Access Licensed Clinicians when the indicated need by the consumer calling exceeds that provided by the Access Qualified Professional.

Accurately documents all contacts according to LME and State standards and promptly transfer screening and triage information to the provider chosen by the consumer to do the assessment and complete the Person Centered Plan.

Database development and maintenance: assists in the development and maintenance of accurate information about the community resources in the 8 county service area; assists in the maintenance of accurate information regarding the provider network; and assists in the development and maintenance of an efficient system for scheduling referrals to the provider network.

Meetings and training: Participates in clinical and administrative supervision as directed by the supervisor. Attends training to develop and enhance clinical and technical skills.  Participates in cross training to learn and teach skills for effective interaction with consumers from all ages and disability areas. Participates in administrative, peer review, quality improvement and other meetings and activities as directed by the supervisor.

Consultation and Care Coordination: Participates in Care Coordination when appropriate or when directed to follow cases. Participates in Consultation and training activities for providers, and all clinical activities of the LME, including development of policy, review of programs, etc.

Minimum Qualifications:
Master's degree in human services and credentials/licensure by the appropriate NC licensing board. Minimum of two years clinical experience that includes crisis intervention duties.

Other:
The employee must be able to work productively and efficiently with a wide variety of consumers, families, agencies, professionals, law enforcement, and others in the community.  Clients and colleagues may be in distress or in emergency or crisis situations requiring speedy assessment and quick intervention. Some clients are at risk to harm themselves or others and this risk must be accurately assessed.  The high volume of cases requires the employee to balance the need to do a careful and accurate assessment of the consumer’s need and situation, with the need to be efficient in the use of time to complete the screening and triage interaction.  The employee must develop and maintain the knowledge and skills necessary to accurately assess and refer consumers from all disability areas, and also maintain a good working knowledge of resources available in the 8 county service area.. Licensure by the appropriate NC licensing board.
