Note on Integrated Care from the System of Care Coordinator Dear Child-Serving Providers, In our efforts to support integrated care, avoid medication errors and provide the best possible care for children, CCNC (Community Care of Western North Carolina) has recommended the following information be shared with the medical physician of any child receiving mental health treatment or that has been hospitalized for mental health crises. Recently, there have been some serious concerns about contraindicated medications (health vs. psychotropic meds) and lack of collaboration between the medical home and mental health provider which results in less than optimum care for our children. More often than not, none of this information is shared with the pediatrician, so they are unaware of team coordination, crisis plans, etc.The following list is a sample of what local area pediatricians indicated they would find helpful in providing the best care:
The goal here is to avoid any miscommunication when it comes to the care of children.Pediatricians receive information at times with no knowledge whatsoever of any additional care a child may be receiving.This will also assist in avoiding duplicity in services. Documentation regarding the child’s team is vital to making contact with responsible parties in the event of any events that occur. Thank you!
