The New PCP format... Details, Details, Details
Hope everyone is having a great Friday.
Below are a few friendly reminders regarding the use of the new PCP format.
- Please address all items in the plan. If the item is not applicable, please indicate this rather than leaving the item blank.
- Please remember to have the consumer, the Q, and the MD, FNP,PhD, or PA sign the plan. WHN requires the signature of an MD, FNP,PhD, or PA for consumers receiving IPRS funds.
- Please develop a crisis plan which includes a proactive, reactive, and crisis contingency component. In short, this plan should indicate who is taking what action during which times under what circumstances.
- Please keep your original documents! WHN has actually gotten phone calls from providers requesting that WHN send back the original PCP for corrections.
