Quick Reminder on Consumer Transfer Process . . .
For consumers receiving IPRS funded services, the staff of the ACCESS
and Service Management Departments of WHN will work with the clinicians
at NVML and will transfer the consumer's clinical home to the new
provider.
New authorizations will be generated from 45 to 90 days and assigned
to the new provider. This process will be similar to the rollover process
used when Case Management and CBS were converted to Community Support.
10 days prior to the expiration of the authorization, the
new provider should submit a SAR to WHN's Service Management Department, along
with the updated PCP.
Please note: For this transition period , WHN will require only the
following on the new PCP:
- the new provider is listed on the PCP
- the service and frequency requested is on the PCP
- the PCP is signed by an MD, PhD, FNP, or PA
- target dates that are current
- a complete crisis plan
In the event you are a provider having consumers transferred from NVML,
please send a list of these consumers by initials and case number to
Marsha Ring or Kim Kuhn at WHN. Please include both Medicaid and IPRS
funded consumers. Thanks for your cooperation during this transition
period.
NEWS Flashes . . .
Effective November 1, 2006, providers submitting a Plan of Corrective
Action to WHN MUST
use the WHN Plan of Corrective Action Submission Form. This form is a word
document which
is available on the WHN website. The following link will take you to the Performance
Monitoring
page on our website: http://www.westernhighlands.org/pr_performance.htm. Scroll
down to the
Forms section and you will find the Plan of Corrective Action Submission form
posted there.
The format of this form will allow for expansion of the boxes in the document.
This form should first
be saved on your computer, filled out electronically, and then mailed to WHN.
Due to mandated timelines,
WHN will not be able to review Plans of Corrective Action that do not use this
format. Please remember
that WHN is REQUIRED to report providers that do not submit acceptable plans
of corrective action to
the NC Division of MH/DD/SAS Accountability Section.
Effective Tuesday, October 17, the authorization numbering
scheme is changing. Authorization numbers will now have the 5-digit provider
number in the "middle" instead
of a combination of letters. For example: provider 12345 could have the following
authorization number - 0610123450001.
