Access and Registration Procedures
To Case Responsible Providers:
The Access Department has made some changes to the Access Screening Form and Procedure for your walk-in, "no wrong door" screenings. Please begin to use these forms (below) immediately, and ALL new screenings MUST be completed according to this procedure as of January 1, 2006.
